Ultrasound revealed.....
-Markedly enlarged left o***y.
-Heterogeneous ovarian. echotexture with peripheral displacement follicles.
-Associated with ascites.
Findings highly suggestive of ovarian torsion

Dandy-Walker Malformation with Ventriculomegaly--

Ultrasound revealed a posterior fossa cyst, absent cerebellar vermis, and marked ventriculomegaly—findings consistent with Dandy-Walker Malformation.

Dandy-Walker malformation is a rare congenital brain anomaly characterized by: Agenesis/hypoplasia of the cerebellar vermis having Cystic dilatation of the fourth ventricle.
Enlargement of the posterior fossa.
Sometimes associated with ventriculomegaly/hydrocephalus.

Clinical Note
H/O: Postpartum bleeding following C/S.

TVS with Colour Doppler:

Cystic vascular lesion noted at the previous C/S scar in anterior uterine wall showing bidirectional flow (Yin-Yang sign), suggestive of uterine artery pseudoaneurysm.
No retained product seen in endometrial cavity.
Mild pelvic fluid collection with internal echoes is present.
Important: D & C should be avoided as pseudoaneurysm may rupture.

Follow-up:
Patient developed severe vaginal bleeding at night and subsequently underwent emergency hysterectomy for uncontrolled hemorrhage likely related to ruptured uterine artery pseudoaneurysm.
