Who should register?
Anyone living in Ontario who does not currently have a family doctor or nurse practitioner.

What is Health Care Connect?
Health Care Connect is a provincial program that helps connect unattached patients with family physicians and nurse practitioners accepting new patients.

Does registering guarantee I will get a provider?
No. Attachment depends on provider availability; however, registering ensures you are included in the process when opportunities become available.

Is there a cost to register?
No. Health Care Connect is completely free.

What information will I need?
You will need:
Your Ontario Health Card
Contact information
Basic information about your healthcare needs
What if I have complex medical needs?

Health Care Connect prioritizes individuals based on healthcare needs and circumstances to help connect those with greater healthcare needs as quickly as possible.

Can I register a family member?
Yes. You may register dependents under 16 years of age. Adults must generally consent to registration.

What if I recently moved to Northumberland County?
You can still register if you do not currently have a family doctor or nurse practitioner.

I am on a clinic waitlist. Should I still register?
Yes. Being on a clinic waitlist does not automatically register you with Health Care Connect. We encourage residents to register with Health Care Connect as well.
