This July, we’re grateful to welcome another batch of qualified PhilHealth YAKAP beneficiaries aged 0–15 years old. Through this program, more children can receive quality eye care, helping them see, learn, and grow with confidence.
Planning to avail of the PhilHealth YAKAP Program? Follow these simple steps:
✅ Step 1: Prepare your PhilHealth details. You can access them through your eGovPH App or request a Member Data Record (MDR) from PhilHealth.
✅ Step 2: Get your PhilHealth YAKAP referral form from the DepEd Schools Division of Baguio or any of the 16 YAKAP partner facilities in Baguio City.
✅ Step 3: Visit Re-Vision Optical Clinic with your referral form and child to receive an eye examination covered by the PhilHealth YAKAP Program
