Hello mga Preggy Mommy, may programa ang MSWDO para masponsoran ng LGU yung pagkuha natin ng Philhealth.
Sharing this STEP BY STEP PROCESS PARA MAKAKUHA NG INDIGENCY SA PHILHEALTH:
Step 1: Pumunta lang kayo sa Barangay ninyo at kumuha ng Indigency para sa Philhealth. Kailangan naka Specify na for Philhealth yung Indigency.
Step 2: Pumunta sa MSWDO (Municipal Social Welfare and Development Office) para mag apply ng Indigency for Philhealth. May mga instances na kailangan kang interviewhin para macheck kung incapable ka talaga na hindi mag bayad ng Philhealth mo. And itatanong din nila kung kailan ang due date mo, inaadvise nila na atleast 1-2 months before ng due date ka mag apply dahil meron itong expiration.
Para masure na maprocess, magdala ka ng mga documents tulad ng Ultrasound.
*Pag pumasa ka sa MSWDO, papuntahin ka nila sa Treasury at Assessor Office para humingi ng certificate. Pero case to case basis, depende yun sa requirements nila.
*Once kumpleto na yung requirements, babalik ka sa MSWDO para mabigyan ka ng Certificate of Financial Incapable.
Step 3: Pumunta sa Philhealth Office,
Need mo magdala ng additional documents (photo copy of Birth Certificate, Ultrasound/Proof of Pregnancy, Valid ID). Bibigyan ka nila ng PMRF form para sa pag update. Once na fillup mo na, ipprocess na nila na yung Philhealth mo at ilalagay na Financial Incapable.
