For the Facebook record, I HEREBY STATE THAT I DO NOT GIVE MY PERMISSION FOR FACEBOOK OR ANY OTHER ENTITY TO USE ANY INFORMATION OR MEDIA OF ANY KIND FOR ANYTHING WITHOUT MY WRITTEN CONSENT.
