He started in 1961 as a sketch artist, drawing for Jean Louis and working as an assistant to Edith Head. Mitzi Gaynor hired him in 1966 for her Las Vegas revue. Carol Burnett and producer Joe Hamilton saw that show and brought him in to dress her new variety hour the following year. He stayed for the entire run. Burnett has estimated he built roughly 17,000 costumes for the series, including the curtain rod dress from the Gone With the Wind spoof that now sits in the Smithsonian.

He met Cher during the first season of the Burnett show, and the partnership lasted the rest of his life, through the Sonny and Cher series, her solo specials, her tours and her Las Vegas residencies. In 1974 he sent her to the Metropolitan Museum's Costume Institute gala in sheer silk beaded to suggest nudity, with feathers at the sleeves and hem. She wore it again on the cover of Time. The look is widely credited as the start of what people now call the naked dress.

He won nine Emmy Awards out of 32 nominations, the first in 1967 for Alice Through the Looking Glass, shared with Ray Aghayan. He was nominated for three Academy Awards, for Lady Sings the Blues, Funny Lady and Pennies From Heaven. In 2002 he became the first costume designer inducted into the Television Academy Hall of Fame. He won the Tony Award for Best Costume Design of a Musical in 2019 for The Cher Show.

He gave a great deal of it away too. In 2005 he auctioned a large portion of his personal archive of sketches, costumes and couture through Christie's and directed part of the proceeds to the Elton John AIDS Foundation. Desert AIDS Project honored him with its Steve Chase Humanitarian Award. His work kept turning up long after the shows ended. Zendaya wore a Mackie gown to honor Cher at the 2024 Rock and Roll Hall of Fame ceremony, and Taylor Swift used Mackie originals on the cover of her 2025 album.
